**UPDATE** Mina will stay at VIMP until 3 p.m. Dr. Smith explained that Dr. Birnbaum is in a procedure and hasnt been able to write Minas discharge orders and give her a final once-over. I called back to find out how much my bill will be and its not completed but its already more than I expected. More than I have in my checking account and I cant make a transfer until July 1. Ive used up all my transfers this month. So, I applied online for another loan to help cover the rest of Minas chemotherapy. I just hope its enough and that I can afford the payments. To say Im despairing today is true.
Mina is at VIMP where shell be until around 11:30 a.m. Right about now, the LVTs are taking her blood and urine to make sure shes fit for todays 90-minute infusion of chemotherapys nastiest, and most effective, drug.
She didnt want to stay and even burped up some puke while we waited for her paperwork. The tech came out with some Litmus paper to test the puke and then cleaned it up for us. Mina was shaking when the tech took her to the lab. I feel bad for her because shes nervous but I know they take very good care of her.
Theyll do a CBC and if her numbers are good theyll give her the infusion. If not, theyll call me to come and get her and reschedule it. I put her on the scale in the waiting room and shes lost nearly another pound this week. I simply cant get her to eat much of anything, despite the mirtazipine. I told them this in the form I fill out every week to let them know how she managed the last chemo.
If she does get the doxo today, its going to be a rough week for her. That shit make her feel nauseated all the time, and she eats even less. But next Tuesday starts her rest week so Im hoping to shovel as much food down her as possible before we start the third quarter.
As for me, Im running out of money and its causing me enormous anxiety. I talked to Dr. Cliver last night, one of Minas regular vets, and I told her that if we both survive the chemo itll be a miracle. I honestly had no idea NONE how difficult this was going to be for Mina and myself. Minas a champ, shes doing the very best she can, but Im the worst cancer caregiver on the planet. I dont sleep much, Im cranky, and I cant concentrate for very long. Last week was supposed to be a vacation, but I swear that Mina was depressed when I didnt go to work yesterday. I think she needs a break from the constant insanity of my anxiety.

Its a good thing Mina is popular with all of her vets because I am certain theyre not so fond of me.
I called VIMP this morning because Mina just wont eat anything in her bowl and she didnt show much interest in my breakfast this morning. She slept most of yesterday but we did take three walks around the property. I dont know where she gets any energy on the very few calories shes taking in.
Dr. Birnbaums LVT explained that when Mina came in for her staging appointment that she was already down to 50 pounds. So to VIMP, shes only lost a little more than a pound and it fluctuates back and forth. Thats why theyre not worried about her weight loss. I explained that in late February when Mina first started rejecting her food regularly, she weighed 54 pounds so, to me, shes lost six pounds overall. At least now I understand why VIMP is so casual about Minas bones sticking out all over.
Dr. B. also called in a prescription for Mirtazapine again, so Ill pick that up today and start her on it tonight. Thats the anti-depressant known as Remeron, that she takes before bedtime and wakes up ready to eat. It helped her appetite back in May so Im hoping it will help again. Cant hurt if it takes the edge off of Minas anxiety, either.
This morning we walked for nearly 25 minutes! Not bad for an arthritic cancer patient of 12.7 years. She wasnt interested in her breakfast, which I served in a ceramic bowl inside her regular metal bowl because my dear friend, Jill, explained that chemotherapy leaves a metallic taste in human mouths and it might be the same for canines. So, I should cook and serve her food in non-metal dishes.
Then, she figured out there are pills in the tofu cream cheese that I made. She spit them out on the carpet, I retrieved them and pushed them down her throat, and Mina promptly heaved em back onto the carpet. I lost my temper and yelled at her and grabbed the pills and pushed them all the way down her throat. Then I sat at my desk and fumed for a bit until Mina came over, tail tucked, eyes wide and sorrowful, nudging my leg with her nose.
I petted her for a few minutes and then decided I needed to get out for some fresh air. I went to the Battlefield Park but that mission turned out to be a bust, so drove to the only store around that sells Earths Best Organic baby food and nearly cleaned them out of second year chicken and turkey jars. That was only eight jars, BTW.
When I got home we went outside for a quick pee and then I fed her two jars of chicken and sweet potatoes and a jar of turkey and vegetables. It keeps her from starving but its not the nutrition she needs. I noticed shed been scratching the wound on her face so we cleaned that up with some iodine and Neosporin and I put more Vagisil on her ears.
Today were going to drive out to Amissville again to pick up our order of Chow Now. Tomorrow, Mina is going to visit Auntie Sue, Uncle Robin and feline cousins Amber and Pearl while I attend a friends vegan wedding. Auntie Sue has crafts planned for Mina, which is usually pretty cool. Last time Mina stayed for the day they made a plaque of pink homemade Play Dough and pressed Minas paw into it, inscribed it with her name and the year and its hanging by a ribbon above my bookcase.

Over-confidence is like a double-edged sword. It can work you your disadvantage. For instance, people who think they are infallible do not find it easy to learn from the mistakes they have made. If you think you are a hundred percent right all the time, chances are, you will lose out in the end. Obviously, if humans are free of self-doubt, we would not be able to develop. All of us have our shortcomings, many of which we dont even realise, and we should be able to rely on those around to point them out to us.
Every now and then, we have to deal with situations that we know needs to be addressed. In such circumstances, we need to express our wishes clearly and boldly. Have you ever tasted food that is so less than ideal, that you swallow but prefer to spit out? Or have you gotten a piece of clothing with faulty stitching or a toy with a missing part that needs to be exchanged? Perhaps you are too shy to demand that which is due to you or simply do not wish to throw up a fuss. Bad service, defective merchandise or poor food can become a measure of your self-confidence, but also of appropriate behaviour. For instance, it is not appropriate to shout at the waiter of a restaurant or the cashier of the store where you bought the defective clothing or toy.
You need to have a clear understanding of the situation. You need to lodge a complaint and have the fault rectified. Then come to the point politely but firmly. If the waiter or cashier does not show any understanding, ask to speak with the manager or the supervisor. More often than not, you will be accommodated if your demands are fair. If you remain calm, polite and firm, you will be successful most of the time.
Getting what is rightfully yours is good for your own self-confidence. In achieving this, you will find you can have a better outcome if you are truly convinced that your interest is just as important as those of others. For example, someone has taken your reserved seat on a plane or train. A polite but firm remark is often the best approach in dealing with a problem. You can politely ask the interloper to let you have your seat. If he refuses, then you must call the steward or conductor or any other official to ask for support. Just remember, youve paid good money for your seat.
Lets say you some out-of-town guests and decide to go to a popular restaurant but every single seat has been taken up. Are you prepared to stand around in front of all the other guests and wait for an empty table, or do you get nervous thing about these eyes looking at you? Insecurity and fear often prevents us from taking an opportunity that can open. Even if it is only to get a table in a restaurant. If you imagine yourself in a more positive manner, you will become more and more self-confident.
Older folks often lose the ability to learn new things or take advantage of what life has to offer. Many have an unhealthy fear of becoming frail or sick, or no longer see that there can be more than one way of getting a job done. They have self-negating thoughts like I cannot do this anymore, or I can no longer cope with this and discouraged themselves. In the long run, they become self-fulfilling prophecies and rob the person of the courage to do things or learn new stuff.
The thoughts that we have in our mind, the power with which it brings forth is so great that they can trigger physical changes, such as premature ageing. Its a fact that those who believe that they have past their prime and are of not much use are certain to age faster than people who refuse to admit defeat. People who believe of their own stupidity in childhood will never be among the smart ones in adulthood. There are techniques that can help a less than average students with their study skills, learning abilities and accelerated learning. Even adults who are swarmed with information can learn memory improvement techniques to help them cope. Older people can play memory games to keep their mind sharp or use memory training to stave off dementia or Alzheimers disease. Everyone can improve their memory to give them a higher sense of self-worth.
Our brain is the most wondrous God-given creation and within each of us lies the Empires of our own making, in our minds. It is what we put into our minds that govern our outcomes in life. In life we need to ask good empowering questions, not poor self-defeating ones. For example, instead of asking yourself, Why am I so stupid? Your mind will come with an answer like re stupid because you never paid attention in class or re stupid because you hang out with stupid friends. But if you ask yourself an empowering question like What can I learn from this situation so that I never make this mistake again, your mind will come up with good empowering answers. With the right thought, self-honesty and discipline, all of us can make a life worth living, rich in our own human experiential learning and growth.

If you were to ask me which muscle group in your entire body you could work to get the greatest benefits in the shortest amount of time, I would tell you without hesitation, the core. Strengthening the core can realize tremendous benefits to anyone regardless of his or her training experience and can do so very quickly.

But what is the core?

The core, as its known in strength training circles, consists of all the muscles in your abdominal and lower back areas. This includes all the abdominal muscles (rectus ab
ominus, internal and external obliques, transverse abdominus and intercostals) as well as the muscles associated with the spine (the erector spinae group) and the hip flexors (iliacus and psoas, collectively known as the iliopsoas).

These muscles all work in harmony to provide stabilization for your body and to transfer power from the legs to the upper body and vice versa. The core muscles also function to keep your insides in, where they belong!

And why is strengthening the core so important?

Weak core muscles contribute to all kinds of problems in the body, the most prevalent of which is lower back pain. By strengthening the muscles that help support the spine and improve posture, you can dramatically decrease the symptoms of lower back pain.

Picture your spine as a column of soda cans stacked one on top of the other. If you wanted to keep that column standing up under stress, what do you think would work better: a bandage (as is used for wrapping injured ankles) or Scotch tape? Sure the tape would keep the cans together but the cans wouldnt receive a whole lot support, would they?

When you strengthen the muscles of the core, you are in effect turning that Scotch tape into a nice, tight bandage, increasing the amount of support that your spine gets.

Core training also has the potential to greatly improve sports performance. Watch a baseball pitcher throw a pitch in slow motion. The power of the throw starts at the legs, gets transferred through the abdominal area (a.k.a. the core) then ends up in the arm where the ball is released. Imagine how much speed and power would be lost from that throw if the core muscles couldnt efficiently transfer the force from the legs to the throwing arm.

The core is the one area of the body that will always give you a great return on your investment.

So how do I train my core muscles?

Exercises that work the abdominals and the lower back are the staples of core training. Also, exercises that target the stabilization and power-transfer duties of the core muscles are very effective.

The most basic abdominal training exercise is the standard crunch. You can go to the following URL to see the proper technique for this exercise:

But Ive got an exercise for you that blows the standard crunch right out of the water. The equipment required for this exercise: one rolled-up towel.

The exercise is known as the Abdominal Sit-Up. It uses a sit-up-like movement but focuses directly on the abdominal muscles rather than the hip flexors (which a regular sit-up does). It is also very safe for your lower back. Another advantage it has over the standard crunch is that it targets the stretched (arched back) range of motion of the abs, which is totally missed in standard floor crunch.

How To Do It:

Lie on your back on the floor. Roll up a towel or mat and slip it underneath your lower back, just above the waistband (the size of the towel affects your body position during this movement - use a fairly large towel).

Your knees should be bent about 90 degrees. Keep your feet close together and knees fairly wide apart. This prevents the hip flexors from having a direct line of pull, helping to minimize their involvement.

Do not anchor your feet or have someone hold them down. This automatically activates the hip flexors. You will get the most out of this exercise by minimizing their involvement.

The difficulty of this exercise depends on where you hold your hands. The hardest position is above your head at arms-length, then beside your head, then across your chest, then straight down between your legs or at your sides. Start with the easiest first then progress to the other positions as you get stronger.

You are now ready to begin.

-Keeping your torso straight and stiff, start the sit-up by
tightening your lower abs then lifting your upper body off the
floor.

-As you continue up, imagine trying to push your face up
against the ceiling (think up, not around).

-When you reach about 25 to 30 degrees above horizontal, hold
there for a second or two and squeeze your abs hard.

-Keep your lower back in contact with the towel at all times
and always maintain tension in the abs.

-Lower yourself down slowly and under control. Do not just drop
back to the ground. The negative portion of this exercise is
extremely effective.

-Remember to adjust your arm position depending on the strength
of your abs (see above).

Incline Ab Sit-Ups

If you are a beginning trainer, this is a good starting variation of the Abdominal Sit-up.

Set an incline sit-up board to a slight incline. If you dont have an incline sit-up board, you can use an adjustable incline bench, a decline bench, a Step platform with a riser under one of the ends or a flat bench with something under one end. You can even use a propped-up 2 x 6 board!

Your head should be on the higher end with your feet placed on the floor.

The execution is exactly the same. The only difference is that the tension on the abs is much less due to the greatly improved leverage in this position, allowing even people feel that their abs arent strong enough to do the exercise.

Lying Superman Raises - An Exercise For The Lower Back

Though this exercise has a rather unique name, it is an excellent strengthening exercise for the lower back that you can do almost anywhere.

How To Do It:

Lie face down on the floor with your arms stretched out directly overhead (like Superman flying, hence the name).

Raise your left arm and right leg into the air at the same time, also raising your chest slightly off the floor. Hold there for a second and squeeze the muscles of your lower back. Lower your limbs back to the ground then raise your right arm and left leg and hold for a second.

You can push down with the hand that is on the ground to help raise your other arm and chest higher off the ground.

This exercise, even though it only uses your limbs as resistance, provides an excellent way to strengthen the lower back muscles.

Conclusion:

These two exercises will give you a good place to start with core training. You can begin improving your core strength by doing these exercises 3 times a week for 2 to 3 sets each.

Make core training a priority in your exercise routine and you will rapidly reap the benefits of having a stronger, more injury-proof midsection and back.

As we set our clocks back and hour we see that time is not real. We only have the eternal moment. So we need to bless and cherish that moment. Each moment is a miracle. To be present in it is a gift from God. That is why it is called the present. smile.
Many of us dwell in the future and in the past. We miss the only moment that is important when we do that, the eternal now. This is the only moment that exists. It is impossible to live in the future and in the past and recognize what is happening in the moment. When we immerse ourselves in the livingness of the now.
When we are doing something that we love we are not aware of time. When I garden I can spend several hours in the yard and not even feel tired. Suddenly I will realize that I have not rested or ate I was so interested in what I was doing. I am that way about writing. I began to research and write and hours pass, even days and I do not feel weary. I am so excited about what I am learning and sharing it that my every thought is in the moment.
In order to feel more at home in the present moment, it is important to try to stay in that moment. Often when someone is talking to us we are thinking about what we want to say or what they are going to say next. That is not showing respect for ourselves or the other person. Focus on what others have to say giving your attention is an act of love for you and the other person.
This may seem like an overwhelming task when we first practice it being in the moment. For we want to judge what is happening, intuit the other persons or our next move, fill in the silences. Being ever present may seem like work at first but as we practice it becomes more and more familiar and comfortable. Becoming a part of what you are doing so there is no separateness is truly being in the moment. It is a giving of love, that flows back to you from the work of the moment. Each person, each task deserves your undivided attention. This is then the love that you are giving and it cannot help but flow back to you. Anything that is loved grows.
What does creative activity mean to you? Creative activity may mean writing poetry, cooking a good meal, quilting, reading a good book or even resting. Creative activity whatever it is walking in nature, bird watching, immerses one in the here and now. At the same time it frees us. We learn who we are in the very act of letting go of who we think we are at any given moment. Spirituality and creativity are related. The creative power comes from our higher self and when we are creating we open the channel to Higher Power. We find ourselves and our Higher Power when we forget everything and live in the moment. Creating and giving value to each moment helps us do that. This moment will never come again so enjoy it for the miracle that it is. Today I will get out of the way of my creativity.

TSHTF. A surgical option, regardless of the problem, is always best treated by someone who has been trained to perform the procedure. You dont want to be patient number one in a survival situation. Finally, while I am explaining how to do this procedure, I only recommend that you attempt this in a post-TEOTWAWKI scenario where there are no other healthcare options. Proceed at your own risk.

The most effective way to treat an ingrown toenail that has not responded to non-surgical treatment is lateral nail avulsion with matricectomy. What does that mean? Lets break it down. Lateral nail avulsion is digging out and removing one side of the toenail all the way down to the base. Imagine the nail is roughly a square. The ingrown part is on the left side for example. About 1/5 of the nail, the left 1/5, is removed from top to bottom. The remaining 4/5 is left completely intact. Macticectomy is the process of destroying the matrix, or root, of the nail. By removing one side of the nail, the pressure is removed because there is no nail pressing on the tissue any more. This also allows the infection to drain. By destroying the root on that side there is a very slim chance of the toenail growing back in that area. Over time the skin will heal and you will be left with a skinnier toenail that is unlikely to become ingrown again. Now how do you do this?

Supplies
Light (a bright headlamp works well. Consider working outside in the bright sunlight.)
Non-sterile gloves
Sterile gloves
10-mL syringe
27 to 30-gauge needle
Lidocaine 1% or 2%
Povidone-iodine solution (sold as Betadine at most drug stores)
Gauze pads
Drape (sterile sheet)
Iris scissors (small, 3-4 inch long scissors with fine, sharp points)
Bandage scissors if desired (scissors with one sides outer edge flattened for protection)
Nail splitter if desired (heavy duty scissors with very short, thick blades)
Hemostats (small device that resembles scissors but has clamps instead of blades) a pair of needle nosed pliers (sterilized) can be used in a pinch
Sterile rubber band if desired

Step-by-Step Instructions
1. Have the patient lie down on a table with their knees bent. Their feet will be flat on the table. Pull up a chair and put on non-sterile gloves.

2. Clean the entire toe with povidine-iodine.

3. Numb the toe with medicine: If you have lidocaine (1% or 2%) without epinephrine, keep reading to learn how to perform a digital block, i.e. numbing, of the big toe.
Note: Make sure the lidocaine does not have epinephrine in it. Epinephrine is a vasoconstrictor, meaning it clamps down blood vessels. This can prevent circulation to the toes. If you stop circulation with medicine, you have no idea how long it will last, and you could kill the tissues in the toe. Your patient wont feel you remove their toenail, but in a few weeks their toe may fall off! Bottom line: Never use epinephrine on the fingers, toes, ears, penis, or nose.

3A.) Load the lidocaine into the syringe. I have no idea what kind of container of lidocaine you will have, but the standard container is a small jar with an injectable, rubber stopper. Remove the cap and clean the stopper with alcohol. Draw back the syringe to draw in about 8-10 mL (or ccs) of air. Then push the needle into the rubber cover. Inject the air into the jar of lidocaine; this prevents a vacuum from forming after repetitive uses. (If the jar is full, you may have fill the syringe a bit at a time so the rubber cover doesnt pop off when you inject a full syringe of air I learned this the hard way!) Invert the jar so the needle tip is completely covered with lidocaine. Draw back the syringe to the 8-10 mL mark. Remove the needle from the jar. Point the needle up. Tap the syringe to get the majority of the air bubbles to the top. Slowly depress the syringe to express the air bubbles from the syringe. Usually a little of the lidocaine will shoot out. It is not vital to remove all the air, just as much as you can.
3B.) Find the MTP joint (metatarsophalangeal joint). The first joint next to the big toenail is the PIP joint (proximal interphalangeal). The second joint, and usually larger of the two, is the MTP it connects the toe to the rest of the foot.

3C.) Find the injection sites. They are about one-eighth inch above the MTP joint (that is one-quarter inch down the toe, closer to the nail). There are three injection sites: one directly on top of the toe, one exactly on the right side, and one exactly on the left side.
3D.) Inject the lidocaine. Always inject a needle perpendicular to the skin. Puncture the skin with the needle and insert to a depth of about 2 mm (skin is about 1.5 mm thick). Pull back on the syringe to make sure you are not in a blood vessel; if you are, you will see a bunch of bright red blood fill the syringe (if this happens, withdraw the needle and try again a little to the side). You will want to inject about 2 mL of lidocaine at each site. This will sting and burn and then go numb.

3E.) Wait. Wait 5-10 minutes for the block to become effective. If need be, you can give another 1-2 mLs if your patient is still feeling pain. When the toe is numb, proceed.

4. Dull the pain with no medicine: If you do not have lidocaine, things are going to be painful. There are topical numbing medicines available, but these are not nearly as effective as an injection. Most of them are in the same family as lidocaine and are mixed with a cream to make application easier. Another option is to try a topical dental pain reliever such as Orajel or Anbesol (these are topical benzocaine), but again this will only take the edge off. A final option, if you have access to it, is ice; cold temperatures can numb a toe pretty well. An ice water (or snow water) bath is likely the safest way to numb a toe; but be mindful that a cold, numb toe is also a sign of frostbite. Its a careful balance, and I would always err on the side of too much pain. Pain will go away eventually, but a frostbitten toe may never heal. Keep in mind, depending on the person and their pain tolerance, your patient may be able to just grin and bear it.

5. Re-wash the toe with povidine-iodine. Put on sterile gloves. Place a sterile drape over the foot. A small hole in the drape to pull the toe through will keep your surgical field clean.

6. Insert the tip of your closed iris scissors under the corner of the nail on the side it is ingrown. Work the tip down the entire side freeing it from the tissue of the toe. If there are no pain medications, this will be very painful. You should now have the entire side unattached.

7. Split the nail into two pieces. Using a nail splitter, bandage scissors, or iris scissors cut the nail from the free end straight back to the base. You should now have split the nail into 2 pieces (1/5 is the side with the ingrown nail; 4/5 is the healthy side). These pieces are still connected at the root.

8. Apply tourniquet. Some physicians use a sterilized rubber band to wrap around the toe a few times. This acts as a small tourniquet to reduce blood loss which makes it easier to see what you are doing. Having done both, I personally like having a tourniquet in place. Remember to use the tourniquet for the shortest amount of time possible to avoid permanent damage (less than 10 minutes).

9. Remove the toenail. Grab the ingrown toenail with a hemostat. Attempt to grab as much as possible with one bite. Pull straight out toward the end of the toe and to the side at the same time (do not pull up or down or twist). If the nail breaks, just re-grab the remaining nail and pull in the same motion as before. No piece of nail should remain. Some other tissues can look like a nail deeper at the root, but the nail to be removed is hard to the touch of your hemostat.

10. Destroy the matrix. There are a few ways to do this. The most effective and the easiest to perform at home is cautery. Cauterize (i.e. burn) the nail forming matrix (root) in only the area where the nail root was removed. This is probably the most delicate part of the whole procedure. The idea is to burn just the root and not the surrounding tissue think of the old game Operation. Cauterize the entire area twice to make sure you didnt miss a spot.

Hi friends.

Another great day with Jonah! It's as if he got the memo about the Non-Herlitz too! We've all had quite the spirit boost. His blood culture remains negative, although the one from his biopsy spot is growing a couple different "organisms." They are typical skin bacteria, though, so it's not systemic, and they don't seem concerned. I think they have him on the Keflex more as a precaution, as he doesn't seem to have an infection at this point. Praise God!!! His lymph nodes continue to be swollen, but it's probably just because his body naturally has lots and lots of drainage.

I have some cute pictures from today but am way too exhausted to post them. Be on the look out for those tomorrow (you know, unless life happens... as it tends to do).

We've increased his Ativan dose in hopes that we will be able to begin weaning him off the morphine, so he can come home on Tylenol. Tonight we did not give Tylenol and Morphine together (as we had been... possibly the reason he kept spitting it up), but gave only Morphine and increased his Ativan dose a little. This seemed to work out great. He was very relaxed and only cried out when we began to bathe him (one limb at a time). But he's clean, people. And smelling like a baby!!! I know he'll be all greasy and smelling of Aquaphor and gauze again, but for a moment it was nice to kiss all over clean skin and take in the baby smell. Hopefully if this combo continues to work, we can slowly wean him off the morphine and be one step closer to home. In one way, I can't wait, and in one way, I'm scared to death, but I know God will be with us. If you had told me three weeks ago that I would have been able to do all I've done in the last 23 days, I would not have believed you. The strength and comfort He has provided have been amazing. I can do all things...

This has been an ongoing saga for way too long, in my humble opinion! And right now Im not even sure when the end will be, but if I dont get this down now, I will forget many of the details.

December 29, 2008
We had taken our girls to Water Aerobics at the YMCA (Stephanie G with her daughter) to get our exercise during the Christmas break. After exercising and spending some time in the hot tub, my Stephanie and I were together in the YMCA's family shower room (I like the privacy in there instead of the main locker room). This is when she said: Mom, would you look at my toe? It was quite red at the top of the nail bed and pus was oozing out from between the nail and the skin at the top of the nail. It is her right big toe that Im talking about. I knew right away that this was beyond the capabilities of Dr. Mom, so as soon as we got home, I arranged for her to be seen at her doctor's office.

It was the nurse practitioner that saw her and prescribed Augmenten 875 mg with warm/hot water soaks with Epson salts. We started the prescription that day and soaked her foot daily as requested.

I checked her foot from time to time and thought things were getting better, so after the prescription ended, I didnt think about it much

Stephanie stated that her stockings were sticking to her toethe pus had returned! So we soaked in Epson salts again. On Monday I called and made an appointment with Dr. Anderson for after school on Tuesday. We soaked again on Monday.

January 20, 2009
Saw Dr. Anderson and was prescribed another antibiotic, Keflex 500 mg and recommended that she see a podiatrist. I had Steve call the podiatrist in Monroe for an appointment.

January 21, 2009
Saw Dr. Brunsman for the first time and he agreed with the prescription she was currently taking, however, instead of soaking in Epson salts, he wanted her to use a cotton ball soaked with a special salt solution directly on her toe for 10-15 minutes twice a day and not to wipe off the solution, but to use a hair dryer to dry the toe, then bandage it and put on a clean sock. This we did.

The toe is still not better! He swabbed the toe for a culture and gave us yet another antibiotic prescription, this time Augmenten 250 mg.

February 4, 2009
Stephanie stayed home from schoolone look at her very pale face told me that she was indeed sick, but no fever. I called and had her foot appointment moved up from 4pm to 1:30. She started vomiting just before leaving for this visit. I was not happy with this visit at all! The culture came back negative and all he could say was maybe getting an x-ray??? I made the decision that going back to this doctor was not an option and getting an x-ray was not happening either. After getting home I made some phone calls to some infection control doctors that the nurse on the Premera phone line listed for me. After talking with some of them, I decided to do a Mom thing and took her to Childrens hospital.

Childrens was a very positive experience! They treated the whole Stephanie, not just the toe. She was given something for nausea and they numbed the toe and cleaned it out the best they could. They tried to get a sample to for a culture, but there wasnt enough. Which made me realize that the culture taken from Dr. Brunsman was probably not valid either. Anyway, we left the hospital with Stephanies 4th antibiotic prescription, this time Clindamycin at 300mg for 10 days, and a medication to stop the diarrhea that the antibiotic causes.

Follow-up with Dr. Coe. He said to continue the prescription and if the toe was not skin tone at the end of 10 days, to keep taking it for another round.

Prescription renewed.

Both Stephanie and I had dermatology appointments for facial reasons and while we were there, I had her look at Stephanies toe and she recommended that she stay on the RX, but to put clotrimazole 1% cream on the toe two times a day to work on a possible yeast infection.

After much searching, we found that this cream is also a cream for athletes foot.

Stephanie comes home from school and says her toe hurts. It is warm to my touch. Mind you, I had been watching her toe from time to time during the bandage changes, and nothing seems to change.

Saw Dr. Johnson (her real pediatricianfinally!). A new prescription, sulfamethoxazole/tmp ds two times a day. Also, stop the cream and just use an antibiotic ointment, such as Neosporin. He also tried to take a sample for culture. Strongly recommended that she see Dr. Rod Moore, who has his office in the same building. Made the appointment for March 19th but if she gets a cancellation will let me know. Per Dr. Johnson, the toe is probably not now suffering from an infection, but tissue growth of some type. I also noted to him that in all my personal observation, there has been no nail growth at all since this started.

Colds are the most prevalent infectious disease in the United States. In traditional Chinese Medicine, colds are classified by the season, the weather system that produces them, and the symptoms. Since colds appear quite suddenly—and are accompanied by chills, sensitivity to cold and wind, clear watery discharge, lack of perspiration and aches that are better in the presence of heat—they are usually classified as a wind illness.
As such, traditional herbal treatments for a cold include warm pungent herbs that induce sweating. If the cold has a different presentation, it will have a different classification and different therapeutic strategy. In Western medicine, on the other hand, we are most often prescribed the same medications or given the same recommendations regardless of the variability in symptoms and presentation. The actual causes of a cold are also viewed differently.
The three most common symptoms of a cold are nasal stuffiness, sneezing, and a runny nose. Adults and older children generally have minimal or no fever. Infants and toddlers often run a fever in the 100 to 102 degree range.
Depending on which virus is the offender, the virus might also produce a headache, cough, postnasal drip, burning eyes, muscle aches, or a decreased appetite—but in a cold, the most prominent symptoms are usually nasal related.
You catch a cold without knowing it—the symptoms only begin in 1 to 5 days.
Usually the first sign is irritation in the nose or a scratchy feeling in the throat,followed within hours by sneezing and a watery nasal discharge. Within 1 to 3 days, the nasal secretions usually become thicker and perhaps yellow or green—this is a normal part of the common cold and is absolutely not a reason
for taking antibiotics. During this period, childrens eardrums can also become congested.
Colds usually resolve in about 7 days, with perhaps a lingering cough for another week. If a cold lasts longer, consider another problem, such as a sinus infection or allergies. (As I mentioned, a cold primarily affects the sinus region. While you may feel tired or have aches, the illness is centered in the nose, and most of the symptoms are above the neck.)
As parents may know all too well, children average 3 to 8 colds per year—most of which occur in the winter. (In areas where there is no winter, colds are most common in the rainy season.) Typically, parents get about half as many colds as their children do, not least of all due to the ease with which they’re spread.
When someone has a cold, their nasal secretions are teeming with infectious viruses. Sneezing, nose-blowing, and nose-wiping are the most common means by which the virus is spread. But you can catch a cold simply by breathing if you are close enough to a sneezer with a cold—or by touching your nose, eyes, or mouth after you have touched something contaminated by someone with a cold.
So, Rule #1 is the most obvious one: Keep your distance, wash your hands frequently and avoid touching your nose, eyes or mouth.
The flu, on the other hand, can be a much more serious illness. The flu is a contagious respiratory infection caused by a variety of influenza viruses. The flu season typically lasts from November to March. It can cause mild to severe illness, and at times can lead to death.
Seniors, toddlers, infants, and people with certain chronic health conditions are at higher risk for serious flu complications. Again, children are 2 to 3 times more likely than adults to get sick with the flu, and children frequently spread the virus through physical contact.
Flu outbreaks usually begin suddenly and occur in the late fall and winter. The disease often spreads through communities and results in an epidemic. During the epidemic, the number of cases peaks in about 3 weeks and subsides after another 3 or 4 weeks. Because schools are an excellent place for viruses to
spread, families with kids at school have more infections than those without.
Most people who get the flu recover within a week (although they may have a lingering cough and tire easily for a while longer). For elderly people, newborn babies, and people with certain chronic illnesses, however, the flu and its complications can be life-threatening.
As with a cold, you will usually feel symptoms 1 to 4 days after you are infected. You can spread the flu to others before your symptoms start and for another 3 to 4 days after your symptoms appear. The symptoms start very quickly and may include body aches, chills, dry cough, fever, headache, sore throat, and a stuffy nose. Typically, the fever begins to decline on the second or third day of the illness.
The Flu Vaccine: Should You Get One?
Many people still feel that the primary method of preventing the flu is to get an annual flu vaccine. Recent published data, however, has shed some doubt on this belief, yielding results that indicate that the effectiveness and benefits of the flu vaccine are questionable—especially for anyone over 70 years old or
with a compromised immune system.
The scientist who led this particular study concluded that the vaccine had marginal benefits, at best, for the so-called high-risk people—children, adults over the age of 50, people with weak immune systems and workers in the health care field. So, although the CDC still recommends the flu vaccine for these people, based on the recent data, these recommendations should probably be changed.
Many physicians, myself included, believe that the 130 million vaccines offered every year represent enormous financial profits for pharmaceutical companies—and that the politics of medicine likely play a big part in the CDC policy recommendations. This becomes even more obvious when you take a moment to weigh the vaccine’s potential risks.
What You Should Know About Flu Vaccine Risks
Yes, the flu vaccine can cause side effects. The most common side effect in children and adults is simply soreness at the site of the vaccination. Other side effects (especially in children who previously have not been exposed to a flu virus) include fever, tiredness, and sore muscles. These side effects may begin
6 to 12 hours after vaccination and may last for up to 2 days.
You should also be aware that viruses for producing the vaccine are grown in chicken eggs and may contain some egg protein, which poses an issue for those allergic to eggs.
While the flu vaccine is now available with and without mercury (thimerasol) as a preservative, the mercury-free version still contains trace amounts of the heavy metal, which are introduced during the manufacturing process. Even so, the amount adds up to less than 0.5 millionths of a gram.
A traditional flu vaccine that contains mercury, on the other hand, contains 50 times as much—or more. The traditional flu vaccine would give a 24 pound child about 25 times the daily amount of mercury that the EPA says is safe. So it goes without saying that, if you still choose to receive the flu shot, you should absolutely request the “mercury-free” version.
If you choose to forgo this particular method, however, you’ll be happy to hear that you have many alternative methods to choose from—options that will boost your body’s own defenses without any compromises to your health.
My health strategy for addressing the flu issue has three basic components—and whether were talking about a cold or the flu, this strategy includes my recommendations for both preventing and treating them. With that said, youll likely be among the lucky (I say, healthy) people who sail through the winter
without catching either, if you start with prevention—by far the best defense.
This includes building a strong immune system, which will not only prevent a cold or the flu, but it will also speed up recovery time if you do in fact get sick.
Recommendation #I: Long-term Prevention Through Immune Support
My leading recommendation here is to supplement with medicinal mushrooms, which are well documented for their ability to enhance immune function and help combat infection, especially on a long-term basis. Medicinal mushrooms are literally immune system coaches—and it goes without saying that it is more efficient to train our immune system when we are not under distress, i.e., already sick.
The beauty of medicinal mushrooms is that they are also of great benefit and my leading choice in times of distress, and when under increased risk of attack, as can happen with an aggressive flu pandemic.
Mushrooms naturally contain Beta glucans, simple polysaccharides (sugar/carbohydrate molecules) that enhance immune function by boosting macrophage activity - your first line of defense. Beta glucans can be cultivated in yeast cells and extracted and added to mushroom products for a booster effect. Thus, when you select a mushroom product be sure to select a product that contains additional Beta glucans for the best benefit.
I typically use herb-enhanced medicinal mushrooms on patients in my practice. These herb-grown mushroom formulas can be combined with additional mushroom products for long-term benefits, such as an overall multi-nutrient and immune enhancer for women and men. (Just be sure that your multinutrient contains vitamin C and zinc, both of which are important allies of your immune system.)
The best way to use medicinal mushrooms is to take them regularly at a maintenance dose (the recommended dosage on the label) and up the dosage when you are at greater risk or when you have a cold. I start them with what is called a loading dosage for the first 2-4 weeks. (A loading dose is double the
long-term regular maintenance dose that follows. At times of distress, or if there is an increased risk for colds and flu, doubling the dosage of medicinal mushrooms is extremely beneficial.)
When I get a cold, or when I am treating a patient with a cold or the flu, I use a mushroom preparation called Immucare as frequently as every hour, at the recommended dose. Within a short period of time, the cold is usually gone. But a similar strategy can be used with other supplements, as well.
For example, tonifying and adaptogenic herbs—such as Astragalus, Ginseng, and Eleutherococcus (Siberian ginseng)—are great choices for immune enhancement. Personally, I often use and recommend Flew Away; a Tibetan based herbal formulation that has shown to be an immunoactive compound in clinical studies. Simply take them as directed on the label.
How Your Daily Habits Can Support You
What you do every day is crucial to a strong immune system—perhaps even more so than supplements in the long-term. And these lifestyle factors aresimple and in all likelihood familiar: Be sure to drink plenty of fresh, clean water (about 8 glasses) and exercise daily. Take a brisk walk outdoors, do yoga, go for a jog. Do what brings you joy—believe it or not, happiness is a great immune enhancer.
But most importantly, you must get enough sleep. At least 40 million Americans have chronic sleep problems, and an additional 20 million experience occasional sleeping problems—a number that is no doubt partially responsible for the many health crises our country faces to day.
Adequate sleep is critical. Your body needs that down time to recharge and repair its basic systems so that you are in top function the following day. If you have trouble sleeping, exercise will help you. Also, go to bed at the same time each night and remove any distractions or disturbances from your bedroom—
remember it is your sanctuary of retreat from the cares and distractions of your busy day.
Recommendation #II: Short-term Enhanced Support and Risk Protection
This ones simple: As soon as people around you are coming down with colds and flu, or whenever you travel in the winter, you should increase the dosages of all supplements I recommend for prevention.
Recommendation #III: Fighting Active Colds and Flu
When You Have Them (or they have you!) In this case, medicinal mushrooms are once again my first choice (in the Immucare product), and the Tibetan herbal preparation Flew Away is my next choice.
That said, you should be aware that not all tonics and adaptogenics are appropriate when you are already sick. According to Traditional Chinese Medicine (TCM), many adaptogenic herbs can drive the illness deeper into your body—and we don’t want that.
Herbs that I do think are appropriate in this case, if you wish to add them, include Echinacea and Goldenseal. Various Chinese herbal formulas such as Yin Qiao San (or Yin Chiao), Gui Zi Tang, Ge Gen Tang can also be effective, depending on the specific characteristics of the flu or cold. But you shouldn’t
take these without consulting an herbalist knowledgeable in TCM herbology.Chinese herbalists have used ginger for centuries as a cold reducer and decongestant. Garlic has antiviral antibacterial properties that strengthen the immune system. A hot drink of lemon, honey and water can help decongest and soothe sore throats.
A popular homeopathic preparation called Oscilloccinum can be used to relieve flu like symptoms.
Finally, when you have a cold or the flu, high dosages of vitamin C, as well as zinc are quite helpful.
Daily doses of Vitamin C can boost the immune system, offering potential protection from viruses and colds. Vitamin C is a nutrient that has been demonstrated to have a substantial impact on human health. Taking a daily dose of 1,000 mg of Vitamin C can strengthen your immune system and that, in turn, can help you avoid colds and nasty viruses. However, when have a cold, I recommend that you take 1,000 mg 3 times per day until your symptoms subside.
Similarly, Zinc helps to eliminate already existing free radicals and helps to prevent new ones from forming. Zinc is also the most important mineral to the thymus gland, which orchestrates the immune function in the human body.

The last thing I ever ever want to worry about is money.

Unfortunately, that's all I've been worrying about.
Stupid thing to worry about.

Bills. Bills. Bills.
Paid my taxes the other day. Now I'm broke.
Utilities are due. Rent is coming up.
Water bill. Some stupid rental registration payment.
Plumbing. Furnaces. Hot water tanks.
Money.
Money I don't really have right now.

Can't wait for tax returns.
Then I won't be having a panic attack every ten minutes.

I want to prove to myself that I can handle this, that I'm mature enough to take care of my own shit. But I'm overwhelmed.

I just have to put the lady pants on and woman up. Its necessary.

So I'm going to just pay all the bills first, then worry about big stuff. Piece by piece.

Last week I attempted to sweat the main water line back onto the line connected to the house. It didn't work and I got frustrated. Apparently, the water from the street to the house was frozen. When I hit the copper with the blow torch, it thawed the water, and cracked the valve stem.
So I had a friend of mine over to take a look at it. He said to call the water company and have them use a street key to turn it off. So the water company came out, attempted to turn it off, and found that whatever valve is in the ground shifted, the hole was flooded with water, they had no record of the water being on, and they can't turn it off.

So I had a plumber come out. He sweated off the main valve, and put a new one on. But he can't turn off the water without digging up the street. So the valve is on a little bit to allow the water to expand and escape without bursting the pipes, but I have to wait to put copper in until it gets warm enough that it won't all freeze up.

Anyway. I'm being boring and melodramatic, mostly because I needed to vent and whine.
It could all totally be so much worse.

You all got what was going on there, right? Flu or even a cold is a viral illness. Antibiotics don't work against viruses. Wine tasting is a technique and sometimes intimidating. But it can also be learned and fun. Obviously, wine tasting is the most important activity that goes along with writing a wine review and scoring a wine. The thing to remember is that a wine tasting is not just about the taste in your mouth.
The best way to deal with intimidation is to attack the situation head-on. If you are starting and you have no frame of reference about wine, it doesnt really matter. Lets take a look at some of the most important things involved with doing a wine tasting.
The first impression is always important and this is true for wines as well. So what is the first impression that you can get from a wine? First take a look at the bottle and the label. Checking out the label is very significant because it can tell you your first thoughts. The bottle shape and color can tell you what kind of wine will be in there, and where the country of origin is. The labels can tell you about wine classification, more about country, and ratings.
Look at the wine. Wine tasting starts with simply looking at the wine. You should look at the wine by pouring it into a clear glass and holding it in front of a white background. This allows you to observe its full color without any background effects.
Color differences can be very subtle, and can indicate many aspects of the wine. For example, colors in white wines give different flavors, or might indicate age. You could also tilt the glass to observe the color of the wines In wine tasting, a purple color in the rim could indicate a young wine, while brown could mean a mature wine.
Smell the wine is the next step. We now know that most of a taste actually comes from our sense of smell. Thats why its so important to smell the wine. You want to really get the aromas to go over your olfactory senses. All the subtle nuances are going to be in the nose of the wine.
Taste the wine. Then, take a sip of the wine and swish the wine around in your mouth. It was first thought that only certain taste buds on the tongue were able to detect certain tastes like sweetness or bitterness. We now know this is not true, so swishing the wine in a wine tasting allows all of your taste buds to experience the flavor.
While youre tasting it, you should be able to develop a first impression, or what tastes are most apparent from the wine. Next, take a breath with the wine in your mouth to get an idea of the texture of the wine light, rich, smooth, or harsh. Last, you can either spit out the wine or swallow it, and get an idea for the wines aftertaste.
Basically, thats it. One thing to remember is the more you do it, the better you will get. And, as you taste different wines your frame of reference will continue to grow. As you get further and further into wine, you will want to attend wine tastings, wine seminars, and holds one parties. But dont forget its all about about having fun.

Now that were approaching Christmas, I would like to introduce some of the movies that many Americans watch around Christmas time. Ill introduce a new movie every day, and Ill start with my favoriteA Christmas Story
This movie is about an older man remembering his 9th Christmas, when he wanted a BB-gun (air rifle) more than anything else. He spends the whole movie trying to convince everyone from his mother to Santa Claus that he should get it for Christmas, but everyone tells him that its too dangerous. The movie is set in 1940 in Indiana, and it does a good job of recreating the lives of children at the time.
During the film, there are many short, funny stories and events mixed in with the main story. The main character goes to a shopping mall where he asks the local Santa Claus for his present. In America shopping centers, there is usually a Santa Village set up throughout the shopping season, where children wait in line to sit on Santas lap and tell him what they want that year.
Also, the family is forced to look for an open restaurant on Christmas Eve when their dinner is ruined by dogs. Of course, almost all businesses close early on Christmas Eve, so the only restaurant available is a Chinese Restaurant! (I did the same thing every year with my family in America, too!)
The movie was made in the 1980s, and ever since, it has been broadcast several times every Christmas season. In fact, on the American TV Station , its broadcast continually for a full 24 hours from the night before Christmas and all that day! You might think thats too much, but its estimated that 1 of every 6 people watch this movie each year, and the most watched showing gets ratings over 10 percent.
Unfortunately, this movie has never been brought to Japan, so there are no ways to watch it, and there is no Japanese version available anywhere. It wasnt easy to get a hold of my copy.

Leptin is a hormone that was discovered in 1994.
